An initiative to promote housing stability and holistic well-being for older adults in Saskatchewan by connecting them to safe housing, essential supports, and healthcare services.

Older adults in transition who need: housing navigation and support to connect with resources

What We Offer
Desc1. Housing Navigation- Guidance on housing options e.g assisted living, long term care, market subsidized
1. Help with applications
2. Home Support and Services - Assistance accessing community home supports and services
3. Healthcare and Income Navigation - Asistance accessing community home supports and medical supports; connection to benefits and subsidy programs
4. Eviction Prevention and Shelter Diversion - Strategies to remain housing or find interim safe shelter

How We Work
1. Referral or intake - older adults, families, or professionals can refer or self-refer
2. Holistic Assessment - We learn about your housing, health and support needs
3. Personalized Support Plan - We co-create a plan with you and coordinate next steps
4. Follow-up and advocacy - We stay in touch and help navigate complex systems
